Course objectives
The subject provides an overview of the complexity of issues related to public health policy and strategy. Based on a historical and ethical perspective, students can understand public health issues with an emphasis on health promotion and prevention of illness, on assistance to persons with various conditions and health consequences through an organized effort of the society. The subject explains the importance of health determinants in human life and the significance of risk factors that negatively impact health and result in disease, influencing the quality of life or increasing mortality. The determiners of worldwide significance are discussed. The significance of prevention and implemented quality in human life is presented as a major social issue requiring focused attention and continuous control. The subject explains health policy implemented to date and the WHO programmes of Health for all 2000, policy Health 21, as well as the European policy for health and well-being for the 21st century called Health 2020, considering their positive influence in meeting programme goals in the CR and elsewhere. Health literacy, as well as health education promoted in co-operation with governmental institutions, the Ministry of Health and health insurance companies belong to important activities related to fostering of public health in the population. The subject also presents the concept of health service and health care provision in the public health care system and health insurance Framework, explaining to students the progress in this are on a national and European scale. The importance of accessibility of health care and health services is stressed for individuals, families and communities, considering the occurrence of social inequalities. The social importance of health and health impairment and its criteria, including socioeconomic criteria, handle the issue in the context of national, European and world-wide demography. History of health care systems is explained, as well as their types, models and formation processes, their positives and negatives, differences, and the principles of solidarity and equity. International comparison is provided. The subject further delineates primary care and the so-called first-contact practitioners, i.e., the physicians who bring the health care closer to citizens, insured persons or patients in their places of residence and workplaces. Conditions for providing fully reimbursed, partly reimbursed and out-of-pocket covered services in the public health insurance system are explained along with legislative processes, organisation of health care, transformation in the CR after 1989 related to privatization processes, and issues concerning medical and non-medical health professions. Mechanisms of quality control in health care are discussed together with the role of the state and health insurance companies, issues related to review medicine and the position of physician reviewers and health care workers entitled to professional audits in the health insurance system. The importance of healthy ageing, quality of life, demand for and consumption of health and social care in old age is explained.

  • 1. Importance of public health, the position of social, assessment and review medicine in the system of public health and health policy.
    2. Health care in an historical perspectives, historical legacy of ethics, nursing care and medicine for the present.
    3. Health and disease - key terms, models, WHO definition, differences in the concepts of health and illness, health potential, well-being, socioeconomic perspective of health, disease/illness and its consequences.
    4. The social value of health, criteria and impact of impaired health.
    5. Significance of health determinants in human life, determinants as risk factors, lifestyle diseases, mortality.
    6. Determinants negatively impacting health worldwide. Principles of lifestyle disease prevention.
    7. Solidarity and equity - significance for the present, WHO definition. Justice in health care and its relation to the formation of health insurance and sickness insurance, social inequalities in a contemporary perspective.
    8. WHO health policy Health 21, Health 2020, health literacy, its national and European importance.
    9. Studies of the health status of populations, the health of national and international populations, a worldwide comparison.
    10. Major features of demographic development in CR and EU.
    11. Health care system models, positives, negatives and differences of health care systems, indicators for international comparison.
    12. Organisation of health care, types of organisation, prevention, health care facilities, health care workers and their competence, education and lifelong learning, ethics in health care, provided health services.
    13. Importance of statistics and information systems in health care, sociological methods, International Classification of Disease ICD-10, classification systems, health registers and programmes.
    14. Formation of the public health system and general and public health insurance, transformation of health policy in CR after 1989, social significance and implications of legislative amendments, privatization, formation of professional organisations, of health insurance and health insurance companies.
    15. Demographic trends in CR in the context of ageing, demand for and consumption of health and social care.
    16. Basic legal acts and subordinate legislation in the system of public health. Legislation and education in health care.
    17. Preventive programmes and activities by health insurance companies aimed at strengthening the health of insured persons.
    18. Quality control in health care.
    19. The role of the physician reviewer in review activities, their influence on the preparation and promotion of health policy in the system of public health insurance.
    20. Healthy ageing and quality of life, demand for and consumption of health and social care in old age.
